Salary: will be in line with HSE salary scale for CNM1 depending on experience

Our client is currently looking to recruit a Senior Nurse Care Manager to join its established team in Dundrum. The Senior Care Manager will report to the Director of Operations. This is a great opportunity to take up a management role in a highly respected homecare company as it enters a new stage in its development. The successful candidate will be joining a friendly team who work hard in a supportive environment.

The main functions of the role are:

  • Lead and manage all Client Services for Dundrum based South and West Dublin Clients.

  • Lead the small team of Care Managers, sharing resource and knowledge with another office

  • Provide guidance and leadership to the team in all matters relating to Client Services.

  • Be proactive in growing the revenue of the branch by managing the recruitment of carers and the acceptance of new HSE and private clients.

  • Manage the start of new Client services from the first point of contact to establishing a quality settled service.

  • Liaise with fellow healthcare professionals including HSE management, local PHNs and other professionals in hospital and community settings.

  • Engage with the HSE in accepting new client referrals optimising existing carer and office resource.

  • Assess the homecare needs of new Clients by conducting assessments in their homes, hospital, or convalescent homes.

  • Communicate the benefits of service in a professional and reassuring manner to Clients and their families/advocates.

  • Create and document tailored Client Care plans in line with company ethos and HSE protocols and guidelines.

  • Start the service for new Clients by matching and introducing Carers with the required expertise and profile.

  • Conduct Client Risk Assessments and oversee handover to the Quality Team.

  • Proactively engage with the Quality Team to avoid and/or resolve any service-related issues.

  • Provide ongoing support for ’existing clients‘ managing medical and HSE-related matters.

  • Interview and recruit new Carers obtaining commitment and loyalty by supporting their Induction training, client introductions and future growth.

  • Support the office team in ensuring that Clients and Carer needs are prioritised.

  • Collaborate with Director of Operations in agreeing monthly and quarterly growth targets.

  • Lead and support Care Management team in achieving growth.

  • Contribute to planning and expansion of Client Services as needed over time.

  • Participate in on-call rota (one evening per week on three month on/off rotation – not including weekends)

  • Other duties as they arise and relate to stability and growth of the branch.

 

Candidates should be a Qualified Nurse (An Bord Altranais or equivalent). In addition, the ideal candidate will possess the following:

  • Previous experience in Homecare/Community based Care Management is a distinct advantage.

  • A strong desire to advocate for the elderly in a community setting.

  • The ability to multitask, to work under pressure and to use their own initiative.

  • The ability to prioritise and problem solve with excellent attention to detail.

  • Have a confident and clear communication style and be able to influence and reassure people from all backgrounds.

  • Be energetic and positive with an ability to judge situations and the needs of vulnerable people.

  • Have a solid knowledge of the health and social challenges facing the elderly.

  • HR / Recruitment experience would be a bonus but not essential.

The office hours are 9:00 – 5:00pm Monday to Friday. Salary will be in line with HSE salary scale for CNM1 depending on experience. Also, you will be paid mileage and expenses. Work laptop and mobile phone.

  • This is primarily an office and team-based role.
